We already discussed about unsafe act and unsafe conditions & its role in an accident . Some unsafe conditions are below , it treated as a high serious level.These unsafe conditions are ;
Improper plant layout
Improper design & construction.
Unprotected protruding or machinery harmful parts.
Improper lighting and ventilation.Bad house keeping.Open or unprotected man holes.Excessive noise & extreme temperature..
In adequate personal protective equipments[PPEs].
The placement of defective machinery equipments,tools,lack of proper training.
Improper facilities for prevent radiation harms.
More ever improper planing & unawareness of safety rules & regulations.
Disobedience from the part of management for implementing government policies .
The above mentioned points are lead to accident or dangerous situation. The Maine contributing factors are below;
The management not take any interest or responsibility for ensuring safety & the management does not give any awareness to the employees about safety rule and regulations,non compliance of statutory reqirements specified by government rules .Lack of attention for the selection of key persons like,managers , supervisors etc.Lack of attention for conduct safety audit & Job safety analysis at frequent interval and safety inspection for strengthen safety.
